The carriers now create those letters for us so you will need to contact your healthcare carrier to request it. They usually get it emailed to you the same day.

Normally Annual open enrollment for benefits occurs in the second half of October. You can contact Benefits Administration Service Center at 855-224-6200 regarding open enrollment or in the event you have a life event such as Marriage, Birth of a Child, Divorce, etc. Each year Benefits Administration sends an Open Enrollment Package to employees and also sends email communication with instructions on how to enroll and the timeline. You can log into the benefits administration website at www.mydetroitbenefits.com to enroll or to view for more information.

Make sure your address is up to date in Ultipro, you can update using Ultipro self-service. Then Contact Benefits administration at 855-224-6200 and speak to a representative. Also make sure you address is up to date in ultipro by contacting Employee Services at 313-224-8584

Payroll (Ultipro / UKG)

Every month an email communication is send out to all employees with the training calendar including dates, times and instructions on how to access the training. You may also contact contact Linda Green at [email protected] with specific questions regarding Ultipro (timekeeping system) training.

To be eligible for Holiday pay, you must have paid work hours or approved Vacation the day before and after the holiday(s), If you have paid Sick day the day before or after the holiday(s) or any unpaid time (incl. unpaid Departmental Leave) off then you will not receive holiday pay.

Every year from December 24th to January 1st, (exact dates will vary depending on what day of the week Dec 24th, 25th, and January 1st fall) generally the City of Detroit not including the Detroit Water and Sewage Department, Detroit Fire Department, Detroit Police Department and Detroit Department of Transportation will curtail all non-essential operations to the maximum extent possible. Each year detailed communication will be sent out to all employees on closure specific dates for the year. There will be dates that are no-pay during this period. Employees will be charged Vacation, compensatory time, or no-pay for the no-pay dates, employees will be permitted to use departmental leave if they do not have time in their Vacation, Compensatory time banks.

The supervisor or manager of the employee must Contact their Employee Services Consultant.

Log into Ultipro and navigate to Menu (Top left of the screen) / Myself / Pay / Income Tax. You can update it yourself on this page.

On July 1st of each year, Payroll will move hours over 160 in your vacation bank to Must use vacation time.

September 30th of each year (Note: All hours in Must use vacation time will be forfeited if not used by September 30th.)

All City appointees and employees (Except TASS employees) shall be eligible for funeral leave without deduction of pay as follows: Immediate Family = 2 days (Immediate family means husband, wife, son, daughter, brother, sister, father, mother, step-father, step-mother, step-son, step-daughter, grandfather, and grandmother.) Relative = 1 day (Relative means a grandson, granddaughter, uncle, aunt, father-in-law, mother-in-law, brother-in-law and sister-in-law.) Additional 2 days: Where such appointee or employee attends the funeral more than 300 miles from the City of Detroit, the appointee or employee may extend the leave by two days, which shall be charged against the appointee or employee's sick leave banks.

Documentation Requirement: The documentation required is a letter from the Funeral Home on their letter head showing employee’s name, relatives name, the relationship, Date of death, date of service and that employee attended the service. It must also be signed by the Funeral Director or their designee. Timesheet coding is done by supervisor. Documentation must be provided to your departments Employee Services Consultant (or) you can drop it off at their office in Suite 316 on 3rd floor of CAYMC.

Sick time can be used for illness from 1 day to three days, inform your supervisor so they can code your timesheet. In the event you take more than 3 days sick time, the supervisor must inform your department's Employee Service Consultant of the date of absence (this is necessary for you to get paid for more than 3 days you are taking as sick days).

Your supervisor may request Dr. Note.

For Jury Duty, inform your supervisor. Your supervisor must code you timesheet and provide the letter from the court showing you served for Jury Duty to your Employee Services Consultant (or) you can drop it off at their office in Suite 316 on 3rd floor of CAYMC.

Provide a return to work letter from your doctor indicating the date you can return to work and if there is any restriction or not, at least a week before your return to work to Leave management [email protected] and your Employee Services Consultant. Call ph# 313-224-8584 and choose option # 7. You will be send to Henry Ford Clinic for review and clearance of your medical note, the clearance from Henry Ford Clinic must be submitted to Employee Services office at Suite 316, 3rd floor, CAYMC. You will be required to get a negative covid test if you are returning back to the office. It's not required for teleworking.

Beginning September 1, 2021 all Fire Department and DDOT employees are to email FMLA and other medical documents to: [email protected] for processing or contact your Employee Services Consultant with any questions or concerns.

Please inform your supervisor as soon as possible. You or your supervisor will complete a First-Aid report and will have you complete an "Employee Accident or Disability Report". Engagement (Employee Survey)

The results of the December 2020 survey have been shared with Cabinet, across all departments and a summary of results was sent to all employees via CCSD. For more information on your department's action plans in response to the survey results, please reach out to your department director or team lead.

A targeted launch for the next survey is planned for Summer 2022.

Employee Assistance Program (EAP)

A voluntary, short-term professional counseling and referral service designed to help employees and their covered dependents (spouse, dependent children, etc.) with personal, family, or job-related issues. It's free, voluntary, and strictly confidential. The EAP can help users to identify, gain control over, and resolve personal issues or work conflicts that may be interfering with job performance and/or daily life. As an employer paid program, services are free. If long term treatment is needed beyond the short-term counseling of the EAP, a referral will be provided and coordinated with health care benefits.

Reach out to your Employee Services Consultant (or) call HR office at 313-224-8584, choose option 7, and request an outside employment disclosure form. Fill out the form and have it approved by your supervisor/manager/department leadership, once approved send a copy to HR. You can see the outside employment policy using the following link. This is only required if you are working an outside job along with your job at the City of Detroit (regardless of during work hours or not)

Complete the Resignation form and return to your Employee Services Consultant.

Inform your supervisor at least 2 weeks prior to your resignation and co-operate with your supervisor / department in the exit process by returning City issued equipment or making arrangements to return to your supervisor.

Provided such application is submitted within one year after leaving City service and provided the applicant meets the other eligibility requirements as set forth in the Civil Service Rules.

Complete it and submit. It will be provided to Recruitment and if there is an opening and you still meet all the requirements of the job, you may be considered and contacted by Recruitment as applicable.

All employees who report to their City of Detroit office or worksite must complete, daily screening before reporting to the office or worksite by access this link, completing and submitting the daily screening.

COVID Testing: Weekly / Random / One Time tests (CAYMC and Walker Williams Rec. Center)

• Weekly and random testing: The MWF (Monday / Wednesday / Friday) COVID Testing days at CAYMC are only for the weekly and random testing. If you are being tested weekly or tested randomly you can request your test site is at CAYMC when the scheduler calls you. The call back number for weekly and random scheduling is 313-251-4488.

• One time tests / Return to work from FMLA, Leave of absence, from telework: One time tests and return to work tests are generally scheduled at Walker Williams. The number for City employees to call is 313-426-7997 (Mention that you are a City of Detroit Employee).

You may also contact your department designee or employee service consultant to schedule your COVID test. Alternatively, you may schedule the test on your own outside of City of Detroit, if you do so, please email or provide your result to your Employee Service Consultant.

Call 313-230-0505 to schedule vaccination and tell them you are a City of Detroit Employee.

Access this link to complete Vaccination Disclosure form online, please attach a copy of your vaccination card on this online form.

Alternatively, you may visit Employee Services office at CAYMC, Suite 316, 3rd Floor to complete the vaccination disclosure form. This form asks for employees to report that they have received the COVID-19 Vaccine. This is a HIPAA compliant request supported by the Department of Labor.

As we return employees to the workplace we will be scheduling weekly testing for un-vaccinated employees.

The data received through the form will be used for the purposes of assessing our workforce uptake of the vaccine and decision-making with respect to the use of other safety measures in the future.
