Provide a return to work letter from your doctor indicating the date you can return to work and if there is any restriction or not, at least a week before your return to work to Leave management [email protected] and your Employee Services Consultant. You will be send to Henry Ford Clinic for review and clearance of your medical note, the clearance from Henry Ford Clinic must be submitted to Employee Services office at Suite 316, 3rd floor, CAYMC. You will be required to get a negative covid test if you are returning back to the office. It's not required for teleworking.

Beginning September 1, 2021 all Fire Department and DDOT employees are to email FMLA and other medical documents to: [email protected] for processing or contact your Employee Services Consultant with any questions or concerns.

Please inform your supervisor as soon as possible. You or your supervisor will complete a First-Aid report and will have you complete an "Employee Accident or Disability Report".
