This document will guide you on how to complete the application accurately to guarantee approval in a timely manner.
Step 1: Applications are available from Elevator-Permit-Forms (for printing purposes only) or by visiting:

ONLY A LICENSED LIFTING CONTRACTOR can get a permit to INSTALL Lifts
The applicant MUST also be registered with the city of Detroit to obtain permits. All fields are REQUIRED in the application EXCEPT WHERE INDICATED. Supporting documentation: MUST provide along with the request for a copy of the current license of the elevator contractor, the plans of modifications to be made, driver's license, photo identification of the driver who is not from the state or passport.
If the applicant has a license from Journeyperson, he / she CAN NOT get permission AT ALL
Note: AN INDEPENDENT APPLICATION IS REQUIRED FOR EACH DEVICE OR TYPE OF INSTALLATION ALL FACILITIES MUST COMPLY WITH THE CITY OF DETROIT ELEVATOR CODE AND WITH CODES A17.1
Step 2: Submit the application with the appropriate documentation and fees (in person or by mail)

Required Documents: Driver's License, Copy of the actual license of the Elevator Contractor, send plans for the installations to be made
Note: Make checks (initial checks are not accepted) a name of: "Treasury City of Detroit". Other methods of payment accepted are cash, credit cards (Master Card, Visa or Discover only), money orders and cashier's checks.

Step 3: The elevator chief inspector of the elevator division will review and approve the application and the documents presented

Step 4: Once the approval has been obtained, the applicant will be responsible for paying all fees associated with the L & P permit counter.
Step 5: The L & P cashier will issue the Permit and a receipt paid to the applicant. L & P Cashier issues the permit to request the same day if it is in person or sends the permission to the applicant if the request is sent by mail
If the request is sent by mail, the receipt with the shipping number will be mailed to the applicant. This permit process takes 5 to 10 business days
If the application was submitted in person, the applicant received the authorization number in person on the same day
