Children's Special Health Care Services (CSHCS) is a grant funded program by Michigan Department of Health and Human Services (MDHHS) that supports children and some adults with chronic illnesses and special health care needs.
Children and youth under the age of 26, regardless of income or insurance status, may be eligible if they meet citizenship requirements, are Michigan residents, have at least one qualifying chronic condition, and require specialty medical treatment.
Individuals of all ages diagnosed with hemophilia, cystic fibrosis and sickle cell disease are also eligible for the CSHCS program.
CSHCS currently covers more than 2,700 qualifying medical diagnoses.
The program provides a holistic, family-centered approach to care through a multidisciplinary team that delivers care coordination, case management, transportation assistance, and referrals to community-based organizations and resources based on individual client needs.
The CSHCS team offers essential education related to prevalent chronic conditions within our region, including asthma, epilepsy, diabetes, sickle cell disease, cardiac conditions and kidney disease to help individuals and families prevent complications and improve long-term health outcomes.
Through outreach activities, program enrollment and renewal support, transportation coordination, and individualized care planning, the local CSHCS program plays a vital role in improving the health, well-being, and overall quality of life for children, youth, young adults, some adults, and their families.
CSHCS helps persons with chronic health problems by providing:
- Coverage and referral for specialty services based on the person's health problems
- Family centered services to support you in your role as primary caretaker of your child
- Community-based services to help you care for your child at home and maintain normal routines
- Culturally competent services which demonstrate awareness of cultural differences
- Coordinated services to pull together the services of many different providers who work within different agencies
